Class Reschedule Request Form

All class reschedule requests must be made before the class with at least 72 hours advanced notice. While we will do our best to schedule a makeup class for you, we cannot guarantee a make-up class given our classes are grouped by scores (students should attend classes aligned to their scores) and we only have a limited number of classes running each week for each instructor.
